Bolle Prescription All-Inclusive Pack - A Simple Turnkey Solution:

1. Complete all of your personal/employer details in the top section of the form
2. Visit your nearest Boots opticians or independent opticians with the prescription form. (Boots Opticians have the Bolle Safety frames available to view and select)
3. Your Optician will complete the technical section of the form
4. The completed form will then be sent to Bolle Safety by your optician for processing
5. When your safety glasses are manufactured, they will be sent directly back to your optician.
6. Your optician contacts you to arrange appointment and advise the glasses are ready for collection and final fitting.
